Instructions for Obtaining Connecticut Medical, Connecticut DEA & Federal DEA Licenses and subscribing to the Connecticut Prescription Monitoring and Reporting System (CPMRS)
Connecticut Medical License
At least six months prior to your start date, you must go on-line @ www.ct.gov/dph to review the requirements and complete the application to obtain your Connecticut Medical License. The State of Connecticut does not accept paper applications. If you have questions, please call the State at 1-860-509-7603.
Please use the hospital’s main address on your application, which is “282 Washington Street, Hartford, CT 06106”. Please rest assured that you will be notified of any correspondence that is sent to Connecticut Children’s on your behalf.
Connecticut Controlled Substance License
In addition to a Connecticut license, a Connecticut CSR (Controlled Substance Registration) is also required. You may apply for your Connecticut CSR after your Connecticut Medical license has been issued. Please go on-line @ www.ct.gov/dph. Questions should be directed to the State at 1-860-713-6065.
Federal DEA License
A Federal DEA license is required indicating a Connecticut address; therefore, transferring your current DEA license may not be a feasible option. Our insurance carriers require a photocopy indicating a Connecticut address before they will accept your applications, so transferring your current Federal DEA will not afford us enough time to have you fully credentialed with our insurance carriers. Also, transferring your current DEA to a Connecticut address won’t allow you to prescribe in your current State, so you will need to apply for a new Federal DEA. It is important to note that you can have more than one Federal DEA license in effect at the same time. Please obtain by going on-line @ www.deadiversion.usdoj.gov. Connecticut Prescription Monitoring and Reporting System (CPMRS)
Once all of your Connecticut licenses are obtained, you will need to register as a user with the Connecticut Prescription Monitoring and Reporting System (CPMRS) @ www.ctpmp.com. You will need your license numbers prior to completing the on-line form. Once submitted, you will receive a confirmation page that must be notarized, and then faxed along with a copy of your driver’s license, passport or government issued photo identification card to 860-622-2608. The State will not accept applications that are mailed. Once your application has been processed, your username and password will be e-mailed to you. Please call 860-713-6073 with any questions.
